Care Design NY Search of Service (CDNY-SOS)
In October, Care Design NY launched a new tool (CDNY-SOS) for our Care Managers to search for service types, organizations and providers in their surrounding areas. As a Care Design NY Network Provider, your organization and its service information is included in this searchable database, therefore streamlining and broadening the Care Managers’ access to services needed for the individuals they support, and increasing referrals to your organization. 

In an effort to make sure our network providers have up to date information in this new tool, we are asking that providers take the time to search for and review their organization’s information HERE. From there, if updates/edits to your organization's information are needed, please fill out the Network Provider Demo Update Form in the link provided with the search results.   You may also always update organizational information as it changes at any time using the Network Provider Update Form.

Working Together to Coordinate Care

The goal of Health Homes is to improve care and health outcomes, lower Medicaid costs and reduce preventable hospitalizations, emergency room visits, and unnecessary care. The CCO/Health Homes are tailored to meet these goals by ensuring person-centered supports of individuals with intellectual and/or developmental disabilities.  

How can Care Design NY help you?

Working together with Care Design NY partner agencies will enhance service integration and coordination for the people we serve through a collaborative approach. Members of the Care Coordination Organization / Health Home (CCO/HH) have a dedicated Care Manager who oversees and coordinates access to all services, including disability, mental health, medical and social supports. 


Some examples of how Care Design NY Care Managers can help providers in serving the individuals they support include: 

  • Linking individuals to identified needed services. 
  • Ensuring consents and other critical paperwork are completed for the individual to establish seamless care transitions and information sharing. 
  • Participating and helping to coordinate an individual’s discharge planning or other transitional care needs. 
  • Scheduling an individual’s follow up appointments. 
  • Identifying and addressing barriers to care or support needs.   

How can providers help Care Design NY and the individuals we support together?

To ensure coordination of care, Care Design NY needs support the individual's team and service providers.  Some ways in which providers can help Care Design NY better coordinate care include: 
  • Notify Care Design NY as soon as possible if an individual you support goes to the emergency room or is admitted to the hospital. If you do not know who to contact – please refer to our resource document HERE.
  • Provide Care Design NY Care Managers with treatment plan and, discharge plans for the Care Managers to reinforce. 
  • Invite the Care Design NY Care Manager to meetings, calls or other information sharing sessions where an individual's needs may be discussed, reviewed or re-evaluated. 
  • Notify the Care Design NY Care Manager when a person’s care or program is being changed.  

     

Provider Webinar

Thank you to those providers that attended Care Design NY's most recent provider webinar on September 24. The focus of this past webinar was the Care Design NY Provider Survey which was sent out this past July. Thanks to our network provider feedback, we identified areas of focus for the coming year which we addressed during the webinar.

Shout Out to our SEMP Providers in honor of National Disability Employment Awareness Month

The Care Design NY network has over 80 service providers who offer Supported Employment services to people with disabilities. Supported employment services encompass a wide range of job readiness, job development and job coaching supports.  SEMP providers are critical to promoting the Employment First Initiative set forth by New York State in 2014. Thank you to the Care Design NY SEMP providers for playing such an important role in helping individuals achieve their employment goals!
